Registration Form	If registering by any method other than online registration, please use this form.
Waiver & Release of Liability Form	If registering by any method other than online registration, please sign and return this form to the Parks, Recreation & Forestry Department.
Concussion Information Form	Please read carefully before registering for programs.
Child Information Sheet	If required, please return to the Parks, Recreation & Forestry Department office prior to the first day of the program.
Authorization to Administer Medication Form	If your child requires medication to be administered during the time they will be in our care, please fill this out and return to the Parks, Recreation & Forestry Department office prior to the first day of the program.
Scholarship Program Request Form	Fill out this online form if you wish to request a scholarship for programs.
New Program Proposal Form	Submit this online form to the Recreation division if you would like to propose a new class for our program lineup.
